Welome to Bleeping Computer, golfus.
Go into the Device Manager and delete your modem. When you restart your computer Windows will prompt you to reinstall your modem. If you know where your modem files are on your computer, select the option to manually select your modem from a list. When the next window pops up select Have Disk. Either insert your modem's software drivers CD or point to the file on your computer that has the installation files.
Once the installation is complete I would recommend you restart your computer, even if not prompted to do so.
