Can someone tell me what the issue is. I need to help my Elderly Neighbors PC

What is the make and model of their PC?
The sceenshot implies that the problem lies with the hard drive (could be software or hardware).
I suggest that you start with these free, bootable diagnostics: http://www.carrona.org/initdiag.html
Post back with the results and we'll move on from there.

You can download and create the bootable diagnostics on another computer and then carry them to the neighbors computer to run them.
You can also check in the BIOS to see if the HDD is being seen there (the screen shot shows the system trying to boot from the CD/DVD just before it gets a disk error.
If the hard drive isn't in the boot order, then the error is (most likely) because there's no bootable disk in the CD/DVD drive
If the hard drive is in the boot order, then there may be a problem with the boot files on the driver - or a hardware problem with the hard drive itself.
