Does it mean it's not working at all ? if I don't update ?
Even with notifications off and no update notice option, I'm keep getting it.
The reason why I don't want to update is because
if updated,  it seems like it's a 30 day trial version.
I only want free version not trial or pay version.
Because I do see that trial letter and everything seems like it's pay to use
and not working unless paid.

You do not say which version of Malwarebytes you currently have, what kind of licence you currently have, or what OS you are running (I'm assuming Windows).

The most recent version of MWB for windows can currently be downloaded here https://forums.malwarebytes.com/topic/208689-malwarebytes-32-now-available/  you can run the installer over an existing installation.

Older non-expiring licenses are grandfathered into the new versions, or if you are on the newer subscriptions they will of course work.

If you are running MWB v2.2 or earlier , MWB will stop supporting it soon so you have little choice but to upgrade. Earlier teething issues with MWB v3 are largely resolved but as the V3 product is more resource intensive as it protects in more ways - older PCs may struggle.

Lots more info here https://forums.malwarebytes.com/forum/41-malwarebytes-3/

(apologies - missed you were looking for free version - Trial version will revert to free after 14 days)

it means it's not up to date whether the software or database version.

You need to update to keep most up to date protection

it does offer a trial upgrade to the paid version, which expires after two weeks and reverts to the free version.

There is no difference between the Free, Trial and Premium download files. The installer/setup for all three are the same. If you are installing for the first time during the install, it will ask if you want a 14-day trial mode. After the trial you can choose to continue using Free or upgrade to paid Premium. If you decide to purchase the Premium version, then you will be provided a license that will activate the product and enable the Premium features. If after 14 days, you don't want the Premium version there is nothing you need to do as Malwarebytes 3.x reverts to manual stand-alone scanner that detects and removes malware when you run a scan.
