Hey everyone,

I am having trouble with some BSOD errors, one of which I believe I have fixed with a BIOS and mother board (Intel DX79TO) driver upgrade.  I saw in a 2014 post where hamluis suggested a few tools for diagnostic reasons so I have installed them.  I will let the results tell the rest of the story:

3 differnt bugchecks in the logs that you submitted.

Please start with these free hardware diagnostics:  http://www.carrona.org/hwdiag.html

Please run this report collecting tool so that we can provide a complete analysis: (from the pinned topic at the top of the forum)   http://www.bleepingcomputer.com/forums/t/576314/blue-screen-of-death-bsod-posting-instructions-windows-10-81-8-7-vista/
FYI - I don't often use the Perfmon report, so if it doesn't work please just let me know.
NOTE:  On problem systems it can take up to 20 minutes for the log files to complete.  Please be patient and let it run.

If you still have problems with it running, there's an alternate tool here (direct download link):  https://github.com/blueelvis/BSOD-Inspector/releases/download/1.0.5/BSODInspector-1.0.5.exe

NOTE:
Please zip up the (.ZIP) files - do not use .RAR or other compression utilities.
.ZIP is the type file that can be uploaded to the forums.

OK sorry folks, I did not read the initial post requirements.  Here are the initial answers and the two files (SysnativeFileCollectionApp.zip + perfmon.zip).

· OS - Windows 7
· x64 ?
· Windows 7 - for about 6 months.  Starting about November 2015 I ran Windows 10.  I realized my BIOS is specific to Windows 7 so I switched back.  The BSOD started under Windows 10 but only playing Fallout 4.  Since switching back to Windows 7 the BSOD have happened more frequently and without playing Fallout 4.
· Windows 7 came pre-installed on the system.
· Age of system: 4 years
· Age of OS installation: about 5 days

· Have you re-installed the OS: Yes, twice.

· CPU: Intel® Core™ i7-3820 CPU @ 3.60GHz, 3201 Mhz, 4 Core(s), 8 Logical Processor(s)
· Video Card: NVIDIA GeForce GTX 1070
· MotherBoard - Intel DX79TO
· Power Supply - XTREME Gear 1000W ATX (XG-H1000)

· System Manufacturer (Custom build from Cyber Power PC)
· Exact model number (COBRAZ Fang III - Cobra)

· Desktop

Just for info, I disabled the Realtek High Definition Audio because the NVIDIA GeForce GTX 1070 also has a sound card.  I am not sure if this causes conflicts and I did not do this with Windows 10, it did it automatically.  The Network Controller issue is something I have been trying to track down a driver for and I cannot find any.  It hardware IDs to:

PCI\VEN_168C&DEV_0034&SUBSYS_662111AD&REV_01
PCI\VEN_168C&DEV_0034&SUBSYS_662111AD
PCI\VEN_168C&DEV_0034&CC_028000
PCI\VEN_168C&DEV_0034&CC_0280

Google shows this is a Qualcomm Atheros AR5BWB222 Wireless Network Adapter but I don't see any drivers for it in my original driver package.

In the interim, I will run the listed diagnostics from usasma, jcgriff2 and John Corrona.

I fixed the link to the hardware diagnostics:  http://www.carrona.org/hwdiag.html

Please run ALL of the tests and let us know the results.

I looked at the link for the COBRAZ Fang III - Cobra, and it comes with an Asus motherboard.

Are you sure that this is the model that you have?

Try this link for a W10 download of the WiFi drivers:  https://www.atheros-drivers.com/download-driver-for-Atheros-chipset-with-id-74-and-Windows10-64bit.html

The memory dumps blame btwampfl.sys - a Broadcom Bluetooth driver

Try these drivers (I don't have experience with this one, so it may not install - if so, just let us know)

Only 217 Windows Update hotfixes installed.  Most systems with SP1 have 350-400 or more.  Please visit Windows Update and get ALL available updates (it may take several trips to get them all).
The actual number is not important.  Rather it's important that you checked manually, installed any available updates, and didn't experience any errors when checking or updating.

As you have disabled the Realtek audio, please re-enable it and update it.  Feel free to disable it after you've updated it.

Please update these older drivers.  Links are provided in order to assist you with looking up the source of the drivers.

DO NOT manually delete/rename the driver as it may make the system unbootable!!!:

usasma,

Mem+Tests have passed.  Driver verifier passed though I did nothing to stress the system. Should I be running Fallout 4 to cause a crash while running driver verifier?  I am guessing if I cause a crash this is preferable to generate a report.  I have been grinding through as many of the Windows updates as I can and found a few drivers.  I have a few things that I would like to see if you can offer a solution on.

I have had several Windows Updates fail and others cancelled.  When I used some of the fixes on windows forums they seem to have made the problem worse.  The updates I that have been sucessful no longer show on the update history so I don't know where I am with updates.

Can you point me in the direction of a solution to updates getting stuck or not showing on the update panel?  At this point I am wondering if going back to Windows 10 would be better?  My BIOS seems to be Windows 7 only.  Will this cause BSOD?

The network adapter was on intel's updates for the motherboard, and it was a Gigabit driver.  The "Qualcomm Atheros AR5BWB222 Wireless Network Adapter" search brought up a lot of dead ends apparently this is a common issue.

I have been forced to uninstall the Renesas Electronics USB 3.0 Host Controller and disable it because I get a consistent BSOD when I try to update it.  I was thinking that this might be an ah ha thing but I wanted to run it by you.

Today I am actively working to find more drivers and updates so I will give a more precise update when I can hopefully finish.

OK so while trying to get all the Windows 7 updates I found what seemed like a good solution to the stalling updates.  Unfortunately it BSOD crashed while updating and I started to get more BSOD.  I have decided to start over with Windows 10.  I will look for the driver (if this is infact the root cause) on that end.  If I have more trouble I will start another topic.  Thank you usasma, you have been most helpful.

W10 is actually easier to work with than W7 is

W7 has known issues with updates (see this post by CKing123:  https://www.bleepingcomputer.com/forums/t/625369/high-cpu-usage-and-endless-windows-update/?p=4075499 )

If the errors recur, please post back in this topic, as it may be a hardware problem.

Good luck!

usasma,

Well I was fairly certain the errors would continue and I was right.  However, they do not happen outside of playing Fallout 4.  Here is the info from the first BSOD since re-installing W10 and FO4.

Also, I have had two "crash to desktop's" with no errors today.  I am not sure if these are indicative of something else.

I have also noticed that W10 has found and installed drivers for the Renesas Electronics USB 3.0 Host Controller; W10 list the device as "Renesas USB 3.0 eXtensible Host Controller - 0.96 (Microsoft)". Under details it's decription is "USB xHCI Compliant Host Controller" using a Microsoft, driver version "10.0.14393.0".  Wanted to make sure that was on the table becasuse it seems odd to me.  Also intel has drivers for this device in a two part firmware update; a front and back.  I have two sets of USB 3 ports, both of which do not work.  The front ports are not physically connected to anything and the rear are fused to the mobo.  When I tried to install the firmware updates with W7, it caused a BSOD everytime.

The Broadcom Bluetooth driver seems to have been resolved as well by W10.  This is a Microsoft driver as well.

In any event take a look and thank you for your help again.

Just from your description, I suspect a motherboard problem.

The fact that crashes continue in the different OS's tends to suggest a hardware problem

And the crashes with the USB 3.0 drivers suggest that there's a problem with the USB stuff (which is on the motherboard).

I would suggest disabling the USB 3.0 ports in the BIOS/UEFI and see if that helps.

Drivers are a tricky subject - and as Windows matures, it gets even trickier.

It used to be that you had to install Windows, then you had to install the drivers (and that was how I got involved in these forums - because I didn't know about drivers).

As Windows matured, their repository of drivers became larger, and installing Windows now results in nearly all drivers being detected and installed from the Microsoft installation media.

This is a bit difficult, as these drivers aren't the most recent - but on the plus side, they are tested with the Windows installation, so they should be stable.

I prefer the latest drivers - but a lot depends on what the problems are and if there are different drivers available that might apply to the problem(s).

Will get to the reports later on today/tomorrow AM.

Gotta go help the wife move furniture to her new office building.......

Your UEFI/BIOS (version SIX7910J.86A.0650.2014.0307.0138) dates from 2014.  Please check at the manufacturer's website to see if there are any UEFI/BIOS updates available for your system.  If you are able to install the update through Windows (without booting from an external drive), then go ahead and update it.  WARNING - if the computer might shut down during this procedure, please don't do it, as this may physically damage the computer and prevent it from booting.
FYI - W8 and W10 communicate more with the UEFI/BIOS than previous versions of Windows, so it's important to ensure that the UEFI/BIOS is kept up to date (and that outdated UEFI/BIOS' may be the cause of some compatibility issues).

Only 3 Windows Update hotfixes installed.  Most build 14393 (1607) systems have more than this.  Please visit Windows Update and get ALL available Windows Updates.
The actual number is not important.  Rather it's important that you checked manually, installed any available updates, and didn't experience any errors when checking or updating.

The memory dump shows as a STOP 0x124 in the WER section of the MSINFO32 report.

The analysis of the memory dump agrees with this.

As such, please refer to my discussion in the previous post (about suspecting the USB 3.0 and motherboard)

Please start with these free hardware diagnostics:  http://www.carrona.org/hwdiag.html

If these all pass, then start with this procedure to help identify the faulty component(s):  http://www.carrona.org/strpdown.html

Usasma,

So taking I to accout the age of the computer and the increasing frequency with which I had BSODs (I started getting them outside of Fallout 4 with W10) you helped me uncover I decided to build a new pc. I am keeping some of the components but most it's a fresh build. A question, I'm considering using the drive from my existing computer as extra storage. I have concerns that some of the errors or problems may persist on the drive if I do this. Should I be concerned, should I take extra steps to ensure the drive is completely empty? Any advice you could give would be great and thank you for your assistance.

Leave the old drive out of the new build.
Once you are sure that the new build is running well (and is stable), then try adding the old hard drive into it.
Be sure to completely wipe the old drive and re-format it.
