How does adware typically work when it comes to knowing browsing habits?

Does adware usually get every url you visit in real time, or do they periodically look into your browser history?

Do they have the capability to check actual history files, like index.dat for Internet Explorer, for example? Index.dat files are stored in other places (like the hidden Local Settings Directory), not really in the Internet Explorer folder, so I wonder if they have the capability or permission to look into those.

I used to have adware.hotbar and mywebsearch, so just wondering how they may have done it.

Side question: If adware is installed for one user, is it normally installed for all users as well?
