How to detect & eliminate rogue emailer ?

System is Win7Pro x64, has McAfee LiveSafe up to date, Outlook 2010 shows nothing in Outbox, Sent Items is clean, so I presume some other app is sending out the emails that we see:

We also see them on our iPhone, but when we disconnect the PC from the network, we no longer get these emails.  Ran Malwarebytes, but nothing found, ran TDSSKiller and ComboFix - no help, ran Spybot S&D nothing found.  Uploaded one of the attachments to VirusTotal.com, but it shows as clean.   Any ideas on how to track this down and eliminate the cause would be most appreciated.
