How Can Phone with No Internet Plan Update with Downloads?

I have an LG phone with T-Mobile pre-paid calling services

The pre-paid plans range in value from like $40 ...$100, etc., but the one I have does not have internet service included/enabled. 

Yet, I occasionally see the phone say it has downloaded an update. How is that possible with no internet service on my plan?

Did I get hacked and someone is faking this?

Most likely you were connected to wifi with internet which allowed the update to take place.

Definitely not.  

 

My home WiFi is password secured and I've never entered in the password into my phone.

You certainly aren't hacked, there would be no point.

Does it say anything specific about the update? It could be a carrier settings update that comes directly from your carrier as opposed to from say the phone manufacturer. And if it comes directly from the carrier, then they could be pushing it "outside" of normal data plan "boundaries". Heck, it could even be a regular OS update from the carrier.
