Permanent battery failure

Am facing a problem with my dell laptop battery. Any time a tried switching on the the laptop. A black screen will quickly appear with some kind of test on it say you're experiencing permanent battery failure, try replacing it. I have tried several solutions but it's seems there is no answer. Can someone help me out of this problem.

How old is this battery?

 

Is the battery showing a full charge?

 

What test is being run and by what?

The battery is not showing full charge.the percentage of the battery keeps on degrading everyday. Permanent battery failure just started only yester night when I switched on the laptop, since then I haven't run any test on it. I have been using this battle for the past one year now.

Please answer my first question above... how old is the battery.

 

Is the adapter you are using the original that came with the computer?

The battery is 12 months old. Yea, am using the original adapter that came with the computer.

Have you checked the output voltage of the power adapter to be sure that it is producing the proper voltage?

 

The secondary voltage (output voltage) is listed on the power adapter.

 

What is the make and model of this computer?
