Random Freezing in Dark Souls III PC -- No solution has yet helped

Dark Souls III will freeze at seemingly random points during a play session, no sound, no movement, can't even toggle caps lock, and each time a hard reboot is required. I'm playing on the lowest graphics settings at the lowest resolution and it still freezes. They don't seem to be happening at any significant moment -- They've happened in fights probably fewer times than they have when I would just be walking or standing somewhere. This isn't bonfire related, as they'll happen even when I'm far away from one. Sometimes I'll get a good couple of hours of play in, and rarely I'll even be able to quit the game normally, but most of the time freezes happen within 10-15 minutes of starting the game. I don't know if this is significant, but my computer once froze when I was just watching a Dark Souls III video on YouTube! I've tried just about everything I've been able to find elsewhere, and I have no idea what to do, if there is anything I can do.


System specs:


Processor: AMD FX-8350 8-core 4 GHz
Operating System: Windows 10 (64 bit)
Video Card:  AMD Radeon R9 200 Series
Memory: 16 Gb
I'm not too familiar with how to retrieve info about crashes and such, but if there's any other info needed let me know how to get it and I'll post it.
I really like this game, but it's so obnoxious to have to reboot my whole computer every 15 minutes.

For the first time tonight the game crashed instead of froze and I finally managed to get an event error logged in the event viewer (none of the other freezes showed up in the event viewer). so here's what the event showed:
