Files being overwritten for no reason

Today I was looking for the Epic Games launcher but I couldn't find it even though I know I have it installed. I used Piriform and this is what I found

Why did this happen? Why did files overwrite other files that weren't even in the same folder? The hard drive also has over 400 GB of available storage as well. Any idea on why that happened?

Did you have at least one if not both games going while ccleaning or defragging or wiping free space or having file explorer doing some kind of copying, moving, etc.?

No. I've used the program last week and since then I haven't done any defragging or storage wiping.

Ok, computer is not mult-tasking on any grand scale; let's wait for others to offer possible causes and solutions.
