I would like to add a questionnaire to a web site that is able to be filled in on-line, but I am having a very difficult time doing so. I did try to embed with HTML and still no success. I would attach the file here so you can see what I am working with but not quite sure how to do it. I have spoken to my Host (Ipage, Weebly Software) but they have not gotten back to me yet and it has been over 24 hours. I need to try and get this site up in running by Monday morning. It is not for me, I am doing it for someone else. Thank you for any help you may lend.

Not in any way do not understand what you want to do.
I've been doing web pages for about 20 years, but does not understand, what You exactly thinking.
