need help w/captcha

Hi, been awhile. I'm fighting this captcha form; it's been a battle (flaws in code)
now all looks fine but it doesn't send to email. What's the obvious prob??

Most email clients will not process a <form>. You'll need to give the user a link to the form instead.

I'm at a loss now. I kept trying and checking all yesterday. Today, a day later, I find 15 perfect emails. Thanks for posting.
