I've seen one PC that's had its OS partition erased, plus its recovery partition.

PC boots to (firmware-based?) recovery procedures which fail.

Removed HDD shows five (5) partitions, one of which is empty OS partition.

Available (free) space shows about nominal for drive capacity.

Am replacing HDD and having to do a tedious reload since HDD not backed up.

Just received report by phone of similar-sounding problem on another PC.

Has anyone else seen anything similar to this?

In my earlier days at a computer company, I would have guessed a major failure of a "net-station download" push.  Today, I guess an OS upgrade attempt is failing to complete.  I'll be listening in.
