BSOD - APC Index Mismatch Wdf01000.sys error message

I have a Dell XPS 13 with Windows 10 (fully updated).  Recently I have been getting a BSOD with the above error message.  It is occuring with increasing frequency -- but with no obvious pattern.  I have run the requested tool and the SysnativeFileCollectionApp.zip file is attached as is the perfmon.html file.  

 

Answers to questions:

 

OS:  Windows 10 Pro (installed July 2015 via download...originally a Win 8 system).  Win 10 download was smooth, no issues on install

x64 based processor

Dell XPS 13 laptop (XPS 13 9333( which is 2 years old 

 

CPU -- Intel i7 4500u

Video Card - Intel HD graphics

 

Any assistance that can be provided in resolving this problem is greatly appreciated.

There are 4 ApphelpHardBlock errors in the WER section of MSINFO32.
I suspect that this means that you have incompatible software installed.
Please check your older software for compatibility with this version of Windows.
There's also 1 ApphelpSoftBlock error - I presume that it's similar to the HardBlock errors in that there's an incompatibility on the system.
 

Please uninstall your Western Digital software - it's wdcsam64.sys driver dates from 2008 and is known to cause BSOD's in some Windows systems.

 

There's a lot of USB stuff (hints, not definite errors) in the reports.  Are you using a USB audio device?

If so, please uninstall it's software and physically remove the device from your system.

If this fixes things, then try installing the latest, W10 compatible driver for this.

If it doesn't fix it, then feel free to reinstall the USB audio device.

 

Are you overclocking?  If so, please stop while we're troubleshooting.

 

Although your BIOS/UEFI seems up to date, please check at the Dell website to see if there is a more recent version available.

You have version  A08, and it dates from 8/31/2015 (this may not be the date displayed on the Dell website - so use the version number during your search).

 

Does Dell support W10 on this system?  If so, I'd suggest getting all of the updates for it from the Dell Support website.

 

Beyond this, I have these 4 suggestions:
1.  Get ALL available Windows Updates.  It may take several trips to get them all
2.  Get ALL available, compatible updates for your OS from the manufacturer's website.
3.  Start with these free hardware diagnostics:  http://www.carrona.org/hwdiag.html
4.  If all of the hardware tests pass, then run Driver Verifier according to these instructions:  http://www.carrona.org/verifier.html

Thanks for taking the time to assist me.  I've reviewed your comments above and have the following input:

 

(1)  I've checked my software and the only program that I can determine might have a compatability problem with Windows is Apple Quicktime.  I've tried to update it, but have not been successful.  Not sure if I should remove this or not.

 

(2)  I removed  the wdcsam64.sys file from the system

 

(3) I am not using a USB audio device of any sort

 

(4) I am not overclocking the system

 

(5) I did check the Dell website and discovered that one Intel driver was back level.  I installed it and, while that was only yesterday afternoon, have not seen the BSOD since then.  

 

(6) Dell does support Win 10 on this system and I have previously installed on relevant updates (and was still getting the BSOD)

 

(7) I keep the laptop fully up to date with all Windows updates.  I was current as of yesterday.  As was noted above, same true for Dell.

 

(8)  I will run the hardware diagnostics on http://www.carrona.org/hwdiag.html and add any additional findings.

 

(9)  I tried to run Driver Verifier earlier.  Immediately after having done so, the system crashed (BSOD) and would not restart.  I had created a system restore point before running the tool and used this to get back to operational mode.  While I was unable to read the results, here's a copy of the minidump file:  https://www.dropbox.com/s/c16u84uj4ikqngt/Minidump.zip?dl=0  Please note that this was done BEFORE I installed the driver from Dell.

Uninstall Quicktime.  I've heard rumor that it's not compatible w/W10.  Worst case, you can reinstall it later on.

 

What USB devices are you using?

 

The uploaded minidumps aren't Driver Verifier enabled - and they're basically the same as the others that we've looked at ( the APC Index Mismatch Wdf01000.sys error )

 

So now we wait and see what happens next.........

I've not had any new BSODs since I updated the Intel driver two days ago -- fingers crossed!

Nonetheless, I've gone ahead and uninstalled QuickTime. I agree that I can get it back if required. As for which USB devices I'm using.... Nothing very exotic. External hard drive and occasionally an optical drive or a small mouse I travel with.

So yes, wait it out and hope that the driver update did the trick.
