The evidence from the above is that your hard drive is coming towards the end of its useful life. 'Smart' is 'PASSED' not 'GOOD' and there are numerous references to 'Old-age'.

 

I would strongly advise that you back up ALL your data to external storage then consider a new hard drive. If there is a recovery partition on this drive it should be possible to copy it to DVD or USB so as to be able to re-install your OS after you fit a new drive.
