SMART output interp (or how to analyze)

Hey guys, would anyone here mind interpreting this GSmartControl output? Or could someone please point me in the direction of a website that shows how to interpret it?

 

I've searched a bit on the internet for some info regarding how to analyze these outputs but not having much luck finding info:
