Hi,

I have a pdf form at the following address http://www.ridgebackrescue.co.uk/Trustee%20Forms/Trustee%20Forms%202014/MNRRSRT_Potential%20Owners%20Form_V4.0.pdf

Is it possible to build a similar form that can be completed and emailed from the web site, rather than be printed, filled in and sent by snail mail. I'm sure it can be done but I don't know how to. Where do I start?

Any pointers and help welcome.

Thanks for your time

Derek Smith

You can use an online form builder to do.  Basically you recreate your PDF with form elements.

A few from doing a quick search:

Thanks Martel,

The form we needed was for an animal shelter. The problem has been solved by a piece of commercial software which provides a comprehensive data base plus a simple way to design forms.

You just described a wordpress plug in.

Good luck with the animals, I have 2 rescued dogs and love them like family.
