How to understan that a malware has got into the sandbox?

While surfing in sandboxed web browser,anything downloaded gets into the sandboxed.Let us admit that a malware has downloaded itself without our knowledge.Then how to detect it if av does not detect it?Or if I run a file sandbox,what are general indication of a malware running in the sandbox?
