See if doing it outside of Windows helps. Download the bootable version of Partition Wizard. Burn the iso file. Boot the disk. Select the USB flash drive. Make sure it is your flash drive and not your internal hard drive or you will lose all of your data. Do a quick wipe (zeros). Then partition and format.
Edit; You may have to put your optical drive as the first boot device in BIOS.

That drive is dead, my friend. I loathe to rid of mine. Got a few laying around I just believe there will be a resurrection day for them. Not that reliable but getting better. Have had new and old flip on me without warning.
Our thoughts go out with this moment of silence....

I agree with bludgard. The drive is dead. But, you can usually find some good sales depending on where you live. I picked up a 16GB PNY for about $7. Just be thankful you didn't have any important data on it.

Warning to whoever clicks on the links on the webpage previous post points to: Got 4 open windows (IE11) hijacked and had to kill them through task manager to regain control. The only other option was to "upgrade my browser" from java....L0L
