Okay, I've decided to seek some help with this. This is a challenge.

Avast detects URL:Mal on many if not most video launches from any browser. Internet Explorer, Firefox, Safari, doesn't matter.

It appears to hook into Adobe Flash Player somehow. Both versions, the one for IE and the one for Firefox & similar browsers. Uninstalling and reinstalling do nothing.

It always references a URL at fwmrm.net. I've seen URLs like

hxxp://2df7d.v.fwmrm.net/ad/1?[etc] and

I've disabled plugins, uninstalled & reinstalled Flash (both versions), reset IE, uninstalled and reinstalled Firefox, created a new Firefox profile, and tried Avast (using the most thorough custom scan possible), MBAM, MalwareBytes' beta rootkit scanner, Spybot, Ad-Aware, Combofix, HijackThis, CCleaner, AdwCleaner, TDSSKiller, Junkware Removal Tool, and RogueKiller.

Nothing so far seems to find or clean it. Anyone who can help me shake this will certainly earn my appreciation and respect.
