Graphics card for Skyrim

I want to play Skyrim and Fallout 3 on my PC.
My current setup:
Windows 7 Premium 64-bit
AMD DX-4170 ~4.2GHz
8192MB RAM
1TB Harddrive
DirectX 11
NVIDIA GeForce 7025/ nForce 630a
As far as I am aware, the graphics card is the only thing holding me back.
If there is anything else I need to upgrade, please let me know.
Otherwise, please advise: Which graphics card should I purchase to run Skyrim/Fallout 3 on High/Ultra settings? (I want to spend within GBP100 (USD$170). 
Also, is installing a graphics card as easy as slotting out the old one and slotting in the new one? Do I need to get an extra fan or something?

No just upgrade the graphic card. Your other specs are fine for the fallout 3 and Skyrim
