remove survey 2014 from my computer

I Have Emisoft active and it got in. Tried Malbytes and no luck. Tried all help sulutions under the heading survey 2014 on line and nothing.

Tell me how you know it is on your computer....how does it make its presence felt ?...pop ups?..?

What is your operating system?....xp?....win 7..?
