Simple fast and dirty? Create a standard HTML form and use some free form-handling service to e-mail you the results. (I think bravenet.com offers something like that.)

The easiest method is to use a form like this. This will use form and mailto in the action to open up new email dialog box on the users computer that they can then just press send. This is by far the easiest way to create a feedback, guestbook, etc form in html.

<html>
<head>
<title>Sample mailto form</title>
</head>
<body>

<form action="mailto:myemail@here.com?subject=Feedback Form" method="post" ENCTYPE="text/plain">
<b>Name:</b><input type="textinput" name="name"><br />
<b>Address1:</b><input type="textinput" name="address3"><br />
<b>Address2:</b><input type="textinput" name="address2"><br />
<b>City:</b><input type="textinput" name="city"><br />
<b>State:</b><input type="textinput" name="state"><br />
<b>Zip:</b><input type="textinput" name="zip"><br />
<b>Email:</b><input type="textinput" name="email"><br />
<input type="submit" name="Send Info"> <input type="reset" name="Reset Info">
</form>
</body>
</html>
