Identifying My Modem and Audio Device

I have 2 desk tops here, Dell Dimension 2350 and a Compact Presario 4403,

 

DELL is running and is also using the ?? modem card from the 4403, . . works fine, but, . .

 

I want one for the Presario and don't recall exactly what I purchased.

 

I am believing it is a Diamond Xtreme Sound

 

but to my knowledge it serves both as a modem for dial up connection and audio device.

 

In anycase I am confused concerning the following Modem information, . . .

 

I think that I had a modem problem several months back and reconfigured several options

 

so if you have any ideas of what I have here I'd like to know whats going on ?

 

Most of all I'd like to know what I have here because it is one card that serves both as sound device and allows the dial-up connection to be connected to the back of the tower. I just want to get the same one for the other PC.
