Need Help Interpreting A Hijack This Log

I've scanned with Ad-Ware/Spybot and F-Secure--to no avail. Desperate times take desperate measures!
So I downloaded Hijack This and Now that I have the log. I'm too scared of removing something critical.
Please help...

I see that your system is unpatched. Please go here and download Service Pack 1a (SP1a) and any other critical updates - then post a new HijackThis log.
