Voicefive survey pop up

I've been getting a pop up inviting me to take a the voicefive survey. It comes up randomly and I've tried to remove it in every way google advised. Ran multiple scans with multiple antivirus programs, opting out, and even typing the address manually into my pop up blocker. Nothing seems to work. I found it in my cookies and when I try to delete it, it won't allow. What else can I do?
