Missing Index.dat/unknown Desktop Icon "initdebug"

A new icon appeared on my desktop maybe 2 days ago, the same time frame as a missing index.dat file from C:\WINDOWS\PCHEALTH\HELPCTR\OfflineCache\index.dat. To my knowledge, I didn't create this icon or move it there from somewhere else. I've never seen this icon before. It's name is "initdebug", the icon looks like the system information computer icon. When I click on it, there is a 10+/- second pause and then the H&S screen appears, blank, with a small "System Information" pane is in front of it resembling an error message, saying "System Information can not open this NFO file. It might be corrupted or an unrecognized version". Then when I cancel that, (X) it (only option), the message disappears and the H&S window goes to.... you guessed it. System Summary. Just where I wanted to go when I clicked on "advanced system info" in H&S. So I can get where I want by clicking this initdebug icon, but something is obviously not right.

In the order specified by the Prep guide, I have emptied temp files/temp internet files/recycler, ran Adaware x 2, Spybot S&D, Webroot, Panda, McAfee virus scan (installed/commercial type), McAfee Stinger. Adaware found one data miner, that's it. Nothing else. I have and always have had McAfee firewall on with windows firewall off. My Windows updates are all... up to date. Following is:

Hello brillo and welcome to the BC HijackThis forum. I see no signs of viruses or malware in the log. It is clean.

The initdebug file is an information file and was created by one of the applications on the computer. Since it uses msinfo32 to view itwhen you try and open it you will start msinfo32. Since the file is 0 bytes you can't read it and hence the error message. Just delete the file.
