I am running Windows XP Media Center Edition version 2002 Service Pack 3 on a Dell Inspiron I6400 duo core processor 1.60ghz with 1.99GB Ram.
over time the system has gotten slower and slower and it's barely usable now. It's painfully slow.

I've run This years Kaspersky Antivirus (paid version), in safe mode and no viruses were found (I updated the databases)
I've run spybot search and destroy, updated the databases and run in safemode.
I've cleaned temp files, degraged, and also looked to remove un-needed programs.
I've eliminated the cashe and temp files in my browsers and have kept startup programs to a minimum.
Any help on this would be great.

Thanks.

Define "slow". EXACTLY what is slow?

What happens in safe mode? What happens with a clean boot?

Please run a full scan with MalwareBytes

What exactly is slow? Sluggish, at a reduced rate, tardy, dull. The computer is slow running, it opens programs slowly. For Example when I opened up the browser I double clicked the icon and it took almost 60 seconds for the program to load. Sometimes tabbing between browser windows is incredibly slow. Programs always load slow.

Safe mode has a faster boot process and as far as I can tell the system seems to operate more swiftly. A cold boot does nothing. I ran Malware bytes and found nothing. I ran it in safemode as well.

If it runs normally in safe mode and if you are fairly certain it isn't a malware issue, you can try a clean boot:

Open msconfig and on the General tab choose "selective startup" (uncheck all three items) and reboot. Does the problem still occur? If not, start adding items back to msconfig one or two at a time, rebooting after each change, until the problem reappears and you'll have identified the offending process. This is clearly a time consuming procedure, but it is the best way to determine if some process loading with the system is the cause of your problem.
After you've isolated the cause, do not use msconfig to permanently disable the process. Instead, if it is a service go to START - RUN and type: services.msc (then press enter) and disable the service OR, if it a program, you can download & run a simple app such as Mike Lin's Startup Control Panel (http://www.mlin.net/StartupCPL.shtml) to enable, disable, or otherwise manage startup programs.

Your hard drive could be stuck in PIO mode.

http://winhlp.com/node/10

There is a vbs script you can download on this page and run it, might fix your issue.

One major issue is that you have less than 10% free on your only hard drive. It's a tiny drive - you should consider replacing it with a larger one.

I just installed quickbooks which is a friggin huge program. My hard drive wasn't anywhere near 10% until downloading quickbooks. After installing quickbooks I noticed no noticable change in performance.

Also, just so I understand how this forum works. I followed the instructions above and posted the snapshot and results of a scan. Should I be working with one forum member at a time, and/or taking all advice into consideration at the same time.

I read some on this PIO mode, and I'm not extremely qualified to diagnose however it mentions a common symptom of PIO is choppy or studdering video which is definately something I am experiencing. The link you provided does offer a lot of info but for me it's too much info. Some of the links within that page are dead, and I believe they are the links that I need. Therefore I am unable to use it.

There is a vbs script you can download on this page and run it, might fix your issue.

This link is to the VBS file it was not dead when I clicked on it. You basically download it and double click it. Just right click on the link and save link as.

One major issue is that you have less than 10% free on your only hard drive. It's a tiny drive - you should consider replacing it with a larger one.
