Since today I have been having some issues with my usb ports, when I insert a device into the usb ports it works fine, however unplugging it and replugging it will result in windows stating the device is empty, yet when I look in disk management I can clearly see the USB drive there.

Any idea what the issue is?

I haven't made any hardware changes recently

Are you following the prompts for safely removing the drive?

I'd try deleting all USB controllers (not hubs, not devices) in Device Manager...then rebooting.
