Please bear with me here as this is a VERY LONG and thorough description of everything that was tried/done up until this point, under the guidance of one of the Malware team members.

Thank you for your time and patients.

The original problems started right after I had a Zeroaccess Rootkit Infection cleaned, with help from these forums.
My Original Post is here: ZeroAccess Rootkit and more

Then as time went on there seemed to be a number of various problems.

1. First, the video driver would randomly crash.
Then later on I reopened the previous malware topic here and began to explain what the problem was, and I received multiple BSOD's that looked like this.

http://www.freeimagehosting.net/18sjh

At this time the Nvidia Geforce 480 GTX was still somewhat operable, but I was mostly running of of my second monitor, which is run by a ATI Radeon 5700.

2. I then was told to reinstall Direct X, as it could be what was causing the problem. At first I had gotten this message when I tried reinstalling Direct X.

http://www.freeimagehosting.net/18sjh

I then decided to run the Full Direct X Installer.

And the problem seemed to have been solved.

But then....

3. About 1 1/2 weeks later I started getting new BOSOD's every time I try to start the computer, and could not even get windows to fully boot.

This was the BSOD shown, which I reconstructed using a great Program called BlueScrenView, and my computers minidump files located at c:/Windows/Minidump

A problem has been detected and Windows has been shut down to prevent damage
to your computer.

The problem seems to be caused by the following file: dxgkrnl.sys

If this is the first time you've seen this stop error screen,
restart your computer. If this screen appears again, follow
these steps:

Check to make sure any new hardware or software is properly installed.
If this is a new installation, ask your hardware or software manufacturer
for any Windows updates you might need.

If problems continue, disable or remove any newly installed hardware
or software. Disable BIOS memory options such as caching or shadowing.
If you need to use safe mode to remove or disable components, restart
your computer, press F8 to select Advanced Startup Options, and then
select Safe Mode.

Technical Information:

Upon further inspection it highlights three drivers that were on the stack at the time of crash.

4. I was then asked to supply a Speccy report.

Here is a link to the Speccy Report.

5. I was then asked to run a "sfc /scannow" and post the results.

The results are as follows.

6. I had also tested if I could boot the computer by physically removing the Nvidia 480 video card, and the computer booted up normally.

After reinserting the video card, the problems returned, along with a new one.

My Bios seemed to have been reset, I am not sure if this was due to me needing a new CMOS battery or not, but after about 3-4 reboots, it went away.

This following is the screen that was shown immediately after boot.

Posted Image

7. I was also asked if the videocard was overclocked, and if it had heating problems.

My answer is as follows
To my knowledge the card is factory overclocked. (I had wanted a non-OCed one, but none were available at the time.)...

The video card I have I bought from Newegg. Here is the exact product:
EVGA 015-P3-1482-AR GeForce GTX 480 (Fermi) SuperClocked 1536MB 384-bit GDDR5 PCI Express 2.0 x16 HDCP Ready SLI Support Video Card

I had also thought at one point that it was a heating issue, but I had stress tested the card using Batlefield 3 on Maximum settings and the card went to 83 Celsius without any problems. I had been keeping an eye on the temperature each time the computer crashed now, and it was always between 70 and 76 degrees Celsius.
(However I could not get the temperatures when the computer started and crashed, because it never even got to load the Operating System.)

Additionally about a few months ago I had tested the card using furmark, and had great results.
8. At this point I was only able to boot the computer into Windows Safe mode with Networking, and could not boot normally, as long as the Nvidia 480 was attached.
Here is the Step by Step explanation of what happens when my computer boots up with the graphics card installed.

1. Shows Motherboard logo
2. Checks for cd's and USB's, if none are found...
3. Checks for Disks/hard drives
4. If they are found it tries to boot using the first in the list
(This case being my SSD)
5. Gets to the moving windows logo
(This is where it tries to load the various drivers.)
6. The logo and screen freeze
7. Get a black screen
8. The computer sits there fore a minute or two and reboots

Additionally, the last time I had this problem, and we had reopened the thread I had first installed the newest driver for the video card which caused the computer to crash immediately after windows started up, and loaded all drivers.

It was not until you pointed me to my old videocard driver that things began working again.
(Up to this point, that is.)
9. I was then asked to
Try following Option 1, downloading and running the Microsoft FixIt here: http://support.microsoft.com/kb/2665946

If that doesn't fix it, double check you have the latest graphics drivers installed: http://www.geforce.com/drivers

If updating to the latest drivers doesn't fix it, try Method 3 in that Microsoft article:

Method 3: Investigate possible issues with visual effects or background programs

Having multiple programs, browser windows, or email messages open at the same time can use up memory, and cause performance issues. Try closing any programs and windows that you are not using.

You can also adjust your PC for better performance by disabling some of the visual effects. Here’s how to adjust all visual effects for best performance:

  • Open Performance Information and Tools by clicking the Start button, and then clicking Control Panel. In the search box, type Performance Information and Tools, and then, in the list of results, click Performance Information and Tools.
  • Click Adjust visual effects, if you are prompted for an administrator password or confirmation, type the password or provide confirmation.
  • Click the Visual Effects tab, click Adjust for best performance, and then click OK.
Note: For a less drastic option, select Let Windows choose what’s best for my computer).
Since I could not boot the computer into normal mode, I was unable to run fix #1 since it required he Windows Installer Service, which is disabled in Windows Safe mode.

I then looked into the drivers. It seemed that they were the culprits, and I still think they are the culprits.

I also noticed information about the TDR causing problems and researched that a bit too.

I posted the following reply:
I did a bit more reading up on the TDR feature that was mentioned in the first link you gave. I have read a number of times that this feature was causing problems for many video cards, and that turning it off, via registry could solve the problem.

This is the possible registry fix, taken from Microsoft's website.
http://msdn.microsoft.com/en-us/windows/hardware/gg487368.aspx

From further reading it seems that this has been causing large problems for many people, using windows Vista and 7. (It originated with VISTA where this feature was first introduced.)

The TDR-related registry keys are located under HKLM\System\CurrentControlSet\Control\GraphicsDrivers.

TdrLevel: REG_DWORD. The initial level of recovery. The possible values are:

        TdrLevelOff (0). – Detection disabled.

TdrLevelBugcheck (1) – Bug check on detected timeout, for example, no recovery.

TdrLevelRecoverVGA (2) – Recover to VGA (not implemented).

TdrLevelRecover(3) – Recover on timeout. This is the default value.

In my case I would set the value to 0.

I already backed up my registry and could manually change this value.

I found more great info about TDR and the many issues that seem to effect NVIDIA cards.

Here is the Link: TDR Problems in WIndows 7
However I never actually went through with the possible TDR Fix, because...
I now know for a fact that the issues are DRIVER related.

I just used driver sweeper to completely remove all NVIDIA drivers, and the computer was able to boot normally, using my Radeon video card.
I am now able to work on this outside of a safe mode environment.
(However I am still pondering the above TDR fix, for when I reinstall all the drivers.)

10. So I began to thoroughly research NVIDIA drivers.

And posted the following information
I found this thread from EVGA to be very informative and interesting regarding NVIDIA and its drivers.

Ultimate Nvidia Driver Guide for Uninstalling and Installing driversets

I also found this guide from Overclock.net

For removing all remaining entries and driver files related to NVIDIA.

THis could be what was causing the issues, since I did not know that a uninstall of the drivers via Remove program files and driver sweeper, DOES NOT, in fact remove everything!!

This guide is an alternative, which removes all the files and registry entries that need to be removed for a successful uninstall. This is useful for upgrading/downgrading drivers or switching to another brand of card.

Guide (Nvidia)

1. Go to Add/Remove Programs

2. Uninstall all Nvidia associated software (May require several reboots)

3. Reboot the computer and log back into windows as normal

4. Go to folder options and select “show hidden files and folders”

The next part of the guide will remove all Nvidia files and folders from the computer.

5. Go to your C drive and remove any “Nvidia” folders if there is any

6. Double click the “Program Data” folder (Normally hidden) and remove all Nvidia folders

7. Go back to C and double click “Program Files” and remove all Nvidia folders

8. Go back to C and double click “Program Files (x86)” and remove all Nvidia folders

9. Go back to C and double click “Users”

10. Access the name of your windows profile and double click “AppData” (Normally hidden)

11. Check the “Local”, “LocalLow” and “Roaming” folders for any Nvidia folders and remove them.

12. Go back to “Users” and check the “Default” folder (Normally hidden)

13. Double click “AppData” (Normally hidden)

14. Check the “Local” and “Roaming” folders for any Nvidia folders and remove them.

The next part of the guide will remove registry entries left by the Nvidia drivers.

15. Close everything down and go back to your desktop

16. Click start, then type “regedit”.

17. Click “HKEY_CURRENT_USER” and extend the folder

18. Click “Software” and extend the folder

19. Remove all Nvidia Folders

20. Click “Wow6432Node” and extend the folder (Within the software folder)

21. Remove any Nvidia folders

22. Click “HKEY_LOCAL_MACHINE” and extend the folder

23. Click “SOFTWARE” and extend the folder

24. Remove all Nvidia Folders

25. Click “Wow6432Node” and extend the folder

26. Remove all Nvidia Folders

27. Click “HKEY_USERS” and extend the folder

28. Click “.DEFAULT” and extend the folder

29. Click “Software” and extend the folder

30. Remove all Nvidia folders

31. Click “S-1-5-18” and extend the folder

32. Click “Software” and extend the folder

33. Remove all Nvidia folders

34. Click “S-1-5-21-1632250243-966907716-928185508-1000” and extend the folder

35. Click “Software” and extend the folder

36. Remove all Nvidia Folders

37. Close down regedit and empty your recycle bin

38. Undo the folder options change to hide the hidden folders

39. Reboot

40. Install your AMD/Nvidia drivers and reboot

11. So I began to follow the previous guides and completely remove all traces of the past Nvidia drivers. But I ran into a few problems.
I have gone through and tried to install various drivers, including the last driver I had installed.
I always go through the guides I posted to make sure no part of the previous install is there.
Then install the driver and restart my computer.
After restarting the computer I now always get this new BSOD!
If I go into safe mode and uninstall the drivers the BSOD goes away.

A problem has been detected and Windows has been shut down to prevent damage
to your computer.

The problem seems to be caused by the following file: nvlddmkm.sys

DRIVER_IRQL_NOT_LESS_OR_EQUAL

If this is the first time you've seen this stop error screen,
restart your computer. If this screen appears again, follow
these steps:

Check to make sure any new hardware or software is properly installed.
If this is a new installation, ask your hardware or software manufacturer
for any Windows updates you might need.

If problems continue, disable or remove any newly installed hardware
or software. Disable BIOS memory options such as caching or shadowing.
If you need to use safe mode to remove or disable components, restart
your computer, press F8 to select Advanced Startup Options, and then
select Safe Mode.

I am really not sure what is going on with these drivers!?
I looked up the erro and found an entry for it, in Microsoft's website

Bug Check 0xD1: DRIVER_IRQL_NOT_LESS_OR_EQUAL

I also found this article about it, however it seems to pertain to WIndows 2003 servers, not necessarily windows 7

You receive a "DRIVER_IRQL_NOT_LESS_OR_EQUAL STOP 0xD1" error message, and your Windows Server 2003-based computer stops responding

Found the following here: Win7 "DRIVER_IRQL_NOT_LESS_OR_EQUAL" Blue Screen When Booting
This is something else that I seem to have found. I am not quite sure how the LAN drivers could be interacting with NVIDIA drivers. But The problem I am having does seem to match their description as I am using an ASUS motherboard. I might go ahead an try just updating my LAN Drivers. I don't see the harm in doing so.

I've found a SOLID fix for this problem.

1) Delete ALL Nvidia drivers.
2) RollBack or UPdate your current LAN drivers, especially if you use an ASUS mobo.
3) Restart
4) Windows will automatically install some drivers to your system for your current graphics card if you use Win 7
This is normal
4a) Run CCleaner <--- See below
4b) Run Driver Cleaner from Guru3D.. <--- See below
5) Install the current Nvidia drivers. At the time of this post it is version 197.** released this month.
6) Restart
7) If no BSOD after restart, Set SLI settings and test.
8) After restart test system for BSOD boot up and run screens.

I highly recommend running a few free programs to help with this:
Nova Bench -- Free system test. Run it often. http://novabench.com/download.php

CCleaner -- Gets rid of your old driver files and reboots for you.. Extremely important for removing Nvidia files.
http://www.piriform.com/ccleaner.

Driver Cleaner: Deletes all left over Driver files, specific to Nvida, Creative labs, ATI..
http://downloads.guru3d.com/download.php?det=745

From what I have found the core reason for the this BSOD is an issue with the REALTEK LAN driver competeing with the Nvidia display drivers fighting for the same territory. Once rolled back or updated i have had 0 issues with BSOD's whether during boot or after boot. I really hope this helps guys. I've researched this problem a ton.

Try it and get back to me!!

Sphinx.

I got a new BSOD, with a new set of problems. :(

12. So I went back to do even more research and found a few more pieces of information. (Things that had fixed these problems for other people.)

13. After running the checkdisk I noticed that a NVIDIA service was still running and that the Nvidia control panel was still there.

Here is the post describing it:
I started following the above guide, and am literally going through just about every file/folder/registry entry, by hand to find anything Nvidia related!

I had just found that nvidia had a service running the entire time, that neither driver sweeper or myself had originally picked up.
I halted the serevice, then rebooted, and then manually removed it from these locations.

C:\Windows\system32\nvvsvc.exe
HKLM\System\CurrentControlSet\Services\nvsvc

Another odd thing I noticed was that the Nvidia Control Panel was still in the "Control Panel".
When I click on it, it says I don't have a Nvidia device installed. I am a bit confused as to why it's still there.

The odd thing is that when I go to Programs and Features, it does not show that the Control Panel is even installed!!

I did not complete the guide as I am still trying to figure out how to remove the NVIDIA control panel.

AS I had mentioned previously. It seems that this issue is pretty widespread. Here is a big microsoft thread dating from 2011 until now.

I also found multiple locations where Nvidia Drivers were installed
I also found additional Nvidia files located in these directories but can't modify them without permission from "system". (However I may be able to do so in safe mode.)

Now why would there be 2 sets of backup drivers? Could it be from past installs?

Therese are the contents of one of these folders. They seem to look identical.

14. I also noticed that the Device Manager still was showing thatdrivers were installed, after I had uninstalled all Nvidia Drivers!!

I also took a look at the device manager and there seem to still be drivers installed for the videocard!!!!
Even after I had uninstalled all the drivers...

Here is the list of all the drivers that are shown under Device manager -> NVIDIA Geforce FTX 480

c:\Windows\SysWow64\OpenCL.dll
c:\Windows\SysWow64\nvumdshim.dll
c:\Windows\SysWow64\nvinit.dll
c:\Windows\SysWow64\nvcuvid.dll
c:\Windows\SysWow64\nvcucenc.dll
c:\Windows\SysWow64\nvcompiler.dll
c:\Windows\system32\OpenCL.dll
c:\Windows\system32\nvumdshimx.dll
c:\Windows\system32\nvinitx.dll
c:\Windows\system32\nvinfo.pb
c:\Windows\system32\nvgenco64.dll
c:\Windows\system32\nvcuvid.dll
c:\Windows\system32\nvcuvenc.dll
c:\Windows\system32\nvcompiler.dll
c:\Windows\system32\nvapi64.dll
C:\Windows\System32\DriverStore\FileRepository\nv_disp.inf_amd64_neutral_d895694e10f22a11\NvCplSetup.exe

How would I go about uninstalling them? Would I have to do it manually? I also wasn't sure about removing some of the dll's for example OpenCL.dll does not seem to be a driver specifically for NVIDIA. If it is a shared driver I do not want to remove it.
What about the C:\Windows\System32\DriverStore\FileRepository\nv_disp.inf_amd64_neutral_d895694e10f22a11, where I do not have system permission?

15. At this point I was told to post my problem in THIS forum, and am now awaiting a reply.

The NVIDIA entry in the Control Panel is probably just a leftover shortcut - as you've found out it doesn't link to anything anymore.

Yes, there's likely two sets of drivers due to two different versions of the driver being previously installed.

You probably see the graphics card in the control panel because Windows detects and attempted to install any hardware it automatically detects (whether the drivers have been manually installed or not.)

At this point, I'm not exactly sure how else to troubleshoot this, as this isn't my area of expertise. It's definitely not malware-related. I would recommend posting the errors/problems you're currently experiencing, and how you've tried to fix them, in the Internal Hardware forum.

I would like to thank anyone you for reading through this and for any help/support you can offer in this situation.

I did some further testing, and physically removed my Radeon videocard and just tried to use the Nvidia. Everything worked well, until I installed the drivers.

After the instillation took place and I was prompted to restart the computer, it immediately BSOD with the following message.

*** STOP: 0x00000116 (0xfffffa800c65c010, 0xfffff88003bba4d4, 0x0000000000000000,

*** dxgkrnl.sys - Address 0xfffff88004b14000 base at 0xfffff88004ab7000 DateStamp

I also noticed that whichever video card is in the top most slot on the motherboard, seems to be detected, while the one in the slot below it, seems to have a "error" in the Device Manager.

I just did a complete reinstall of the operating system.

Again the videocard worked fine without the drivers, but the second the drivers were installed I immediately got BSOD's!

I am really getting frustrated here.. I feel like I have run out of options.

After further testing I found that I have an odd situation.

If I put my Nvidia 480 in the TOP PCIE slot, the monitors attached to it are detected, but the monitors connected to the Radeon HD 5770 are not.

I have tried putting the Radeon HD 5770 in multiple PCIE slots below the Nvidia 480.

The same thing happens in reverse. If I put the Radeon HD 5770 in the top most PCIE slot then the monitors connected to it are detected, but the monitors for the Nvidia 480 are not.

Additionally, I found that the Nvidia 480 seems to run just fine in 800x600 resolution using generic Microsoft 6.1.7600.16385 drivers, but will BSOD the second I install any Nvidia Drivers.

This really sounds like a hardware issue with the GTX480.

I read through all of the info but did not see any GPU temperatures. Did I miss them? If not, can you post them?

Also, have you physically examined the GTX480 for any burning, damaged components or fan stickiness?

Have you tried different power plugs from the power supply to the card (I think those take both a 6 pin and an 8 pin plug)?

I have often seen problems like this where a damaged card works fine until the drivers try to access the advanced features or modes other than standard VGA (such as when booting or waking from sleep).

Options for troubleshooting if it is indeed the card are kind of expensive :(
1. Try another GTX480 in the same computer and see if it works better.
2. Put the crashing GTX480 in another computer and see if it works better.

The temperatures, have always been between 70-77 degrees Celsius. When playing a game the card used to sit around 78 degrees Celsius.
(Since this is a 480 I knew these temps were suppose to be ok, since it naturally runs MUCH hotter then most cards.)
The Highest temp I have ever seen the card get to is 83 Celsius, and that was just during Furmark testing.

Before I wiped the computer I used to have a monitor running programs such as afterburner, so I always knew temps fan speed gpu load etc.

The odd thing is that I can not currently get the temperatures as the GPU is showing up as a "Standard VGA Graphics Adaptor". I believe the drivers need to be installed to get temp readings. But since the comp BSOD's with drivers installed I can not get the current temperatures.
______________________________________________________________________________________

I have physically examined the card and there seems to be no damage. Even after manually removing the outer plastic casing, which serves as a wind tunnel for the FAN. The fan itself also works perfectly.

I also cleaned it with a compressed air can, after inspecting it to remove any additional dust that may have built up, and wiped down the fan blades using dry tissues.
(I have not completely taken the card apart, since I do not want to risk damaging it.)

______________________________________________________________________________________

I will try using different power connectors. I have not tried that yet

______________________________________________________________________________________

I will see if I can find a friend who will allow me to try out the graphics card in their computer, as my other smaller rig only has a 350 watt power supply, and can not handle this card.
