transferring data on new computer

I'm going to be buying a new computer, because the old one(real old)just is maxed out I believe? But, I've been afraid to buy the new one because I just don't know how to transfer all the data I have on my old one to the new one.
Please understand I don't have much experience in computer knowledge. So, I pretty much need to be walked through by the hand. Sure would appreciate someone's help. Thank you
