Removal Of Spyaxe Remnants

I am attempting to help my cousin who, due to some serious medical issues, did not do a good job at protecting his computer. He was getting the message "Warning your computer is infected press here for help". He also had a pop-up from the task bar indicating that windows had detected an infection and to click on the icon to view the recommended tools to fix the problem. The first issue was an attempt by me to remove SpyAxe resulting in windows failing to load explorer with an error message. I fixed that problem by going back and restoring a WMF file. I then proceeded to run Spybot, AdAware, SpywareBlaster, and Spyware Doctor. This fixed the pop-up from the task bar and left me with a 2 1/2" by 2 1/2" window with the warning information. I then right clicked on the window and was able to erase the printing and black backround using notepad. I am now stuck with the white 2 1/2 by 2 1/2 window on the upper left hand corner of the monitor. My question at this point is: What is the best procedure to use to remove this white window? I have read the information on smitRem and HijackThis.
