nvidia 450 gts dual monitor issue

I'm not able to setup a dual monitor display with a tv

the way I have it setup:
the monitor is connected to the geforce via dvi cable
the TV=samsung is connected to the geforce via hdmi

I use the nvidia control panel, it recognizes the samsung, I enable it, hit apply, the samsung reads no signal
I take out the dvi cable, leave only the hdmi to tv connected, power comp on it boots up just fine, im able to get a display. I connect the monitor, the screen goes black on the tv, the monitor reads no signal.

I tested with two different hdmi cords both does not work. the hdmi cords I tested works on different devices (xbox,ps3)
I rolled back the device driver update for the graphic card same issue occurs, tv reads no signal with both hdmi and dvi connected

os: win- vista home premium
cpu amd athlon 64 x2 6400
mobo: Gigabyte m57sli-s4
memory : a data technology 333mhz 2gb
video: geforce gts 450 1gb driver version 8.17.12.7533
monitor: acer al2016w 20"
tv: samsung led lcd 40"
psu: 750W
