Permissions/user account issue

I've been having some kind of user account or permissions issue... I believe it originated about 12 months ago during a malware/virus issue I was having and something got damaged during the removal.

Concerns:
1) Red x on network icon - "Connection status: unknown" despite internet connectivity. See http://support.microsoft.com/kb/945925.
2) Windows Update hangs on installation of updates: specifically Hotfix KB947821.
3) Attempted to create a new user profile and copy over everything as described in http://windows.microsoft.com/en-us/windows-vista/fix-a-corrupted-user-profile. Didn't solve problems. Went to retry thinking it was worth a second try and now clicking "Manage another account" hangs and doesn't open user profiles.
4) MBAM/Avast/ESET/Windows Defender/Adaware/BitDefender clean scans.
5) Problem with Windows Installer / unable to uninstall or install any programs (unsure if it's related but iTunes suddenly started saying it needed to be reinstalled to recognize iPods etc. - iTuneshelper.exe seems fine and active)
6) HiJackThis log listed an explorer.EXE file (caps intentional) - googling seemed to indicate this could be a concern.

No vista cd came with computer. Open to any ideas - I've neglected to address the issue for 11 months by turning off UAC but wife's computer just needed a reformat and I'd like to avoid that.
