File Folder Issues

Hello. I have a Windows Vista Studio Computer system. When I attempt to rename or delete a file, my system freezes. The only way I have been able to complete the process is to ctrl+alt+del. When I return to check and see if the file has been renamed, it has, and the deletions are done; but when the box that pops up to verify that deletion is being done will not close on its own. I have to repete the ctrl+alt+del process.

I have done a cmd scan and it stated that I had some folder issues, but it could not correct them all.

One more thing: I tried to complete a scheduled Windows Update this morning and I received an error. The error code was 646. I am a amateur and don't know much about any of this. Thank you for any help you can give.
