Transferring contact lists from one email program to another

I am having some trouble exporting and importing contact lists from one email to another email program. The mapped fields just seem to be all different and this seems to make things not work. I am shocked that there is no program to convert a CSV file into the right format when transferring email clients. I know I can make the adjustments in Excel, but that takes time and I have to think there is an easier way. Is there a progression of exporting and importing in a better sequence to make this work.

Outlook - hotmail, gmail, thunderbird etc...

I would appreciate any help on this topic. Thanks.
