Rootkit TDSS infection

Looks like I have a rootkit infection. TDSSKiller and MBAM diagnose and clean the problem, but it comes back on reboot. I've attached my ark and attach logs. My DDS log is posted below. I've also downloaded combofix and I'm ready to run it as soon as I get the green light.
