OS - Windows XP Home Edition with SP3

I got this computer to fix and it's loaded with malware, these are the symptoms.

1. All the file associations are gone, when I stick in my usb stick with Dr web, Dr. Web is modified and isn't able to run and it gives the "please choose a program to open with" message.

Every single antispyware and malware program cannot run except portable superantispyware which I got to work, I did a quick scan and it found about 2700 infections, however upon reboot the issue with the file extensions sticks pop up.

Also, I get "Windows bad image" file and it gives about 8 to 10 programs which windows has found a bad image, when I mean bad image I mean a message like this...

These are run.dll's that aren't able to run.
