Known person is hacking my computer.

I have had a hacker messing me me for years. I recently set up a firewall rule in Norton 360 to log his attempts to get into my computer. I believe he was behind the eyewonder I posted in Am I infected?
I captured screen shot of his logged attempts. How can I get this guy to stop!

If you have his IP address you can go to this site :

https://www.arin.net

In the upper right, enter the ip address to search the whois. You will then find the name of the ISP that owns that IP address. You can then submit an abuse report to that ISP.

You should also contact the authorities via the links on this page: http://www.justice.gov/criminal/cybercrime/reporting.htm
