Is this a DDR or SDRAM chip?

It's both! As JUICYBoy says, it is DDR, but DDR is a variety of SDRAM (Double Data Rate Synchronous Dynamic Random Access Memory).

And what does 16Mx64 mean?

It explains the arrangement of the memory on the DIMM - a matrix 16 Megabits deep and 64 bits wide (data width).
