Anyone familiar to webhostingpad's cpanel?

I'm looking for someone that is familiar with webhostingpads cpanel and could give me steps on how to create a form, what I want to do is make a newsletter signup page all I want is there e-mail. I'm using microsoft publisher to build my site, tried using one of there forms and I get a 500 error when I test it out and hit submit. I tried using phplist form generator and it does the same thing? Anyone now what I'm Missing? Thanks for the Help a head of time.

Can you post the code of your form ? HTTP 500 means internal server error, so you should contact web host people if they can solve your issue.

I got it fixed, I had some database fields entered incorrectly and a link was broken, that refereed back to the host. Thanks for the tip.
