WS2_32.dll Trojan

Looking for a little help here. I've been struggling with what started as the Internet Security 2010 bug. I've been through a 4 month battle here and it seems to finally caught up with me. I'm now at a loss for a next step. I've got major stability issues though the machine is currently usable if I'm careful about what I do. I'm looking for a little guidance for removal if possible. Thanks for any help you can provide. I've searched through here but it seems that a guided approach is probably best (since you can see my success so far has been less than spectacular).

WS32_32.dll errors with avast! (was McAfee until a recent switch which is when this all spiraled out of control)

Way back when, I did remove this with Malwarebytes... but apparently the process didn't fully rid me of the beast.
