having issues after removing malware

I recently spent a couple hours cleaning up Antimalware Doctor from my pc. After around 10 attempts (using Rkill and Malwarebytes/a free version of CA Security Suite. Malware gone!!! Yay! But after rebooting, my computer will open files and view pictures etc. but won't run any programs. It also won't shut down or restart from the start menu or alt+F4. Any help would be greatly appreciated.

You could try running the Exe File Association Fix at http://www.dougknox.com/xp/file_assoc.htm.

I tried but i can't run anything. Important files are backed up, and I'm going to wipe the HD and start over fresh.
