AntiMalware Doctor/ PEV.RKEXE problem

This is on Vista Professional. I am infected by the Anti Malware Doctor. I have read the recommendations on how to remove it, but I cannot even get the RKILL.COM program (and all its 6? flavours) to work, since everytime I start any of them a box pops up asking me with which program the file PEV.RKEXE should be opened.
Meanwhile MalwareBytes does run, (full scan) but doesnt find anything. Its date is 2/23/2010, dbase version 3780, 195918 fingerprints. The 'update' button is lame. I could download it on another pc and burn it, but I do not know where to find the latest fingerprints file. Any help would be much appreciated.
