You're running really low on a free space.
Since you're running Vista, 14.5GB hard drive is simply too small. In Vista's case, just Windows folder can reach 15GB in size.
I strongly suggest, you buy a new drive, since they're very cheap these days.
Whatever cleaning you do with 14.5GB drive, after a while, you'll run into very same problem.

I agree with Broni's post. With just the files for Vista and the the swap space you do not have any room left at all.

 That explains why the disc space is not freed after running "clean-up " & "defrag" utilities.

I'm surprised, defrag even works, since it normally needs 15% of a free space.
I have to also warn you, that some computers with really low amount of a free space, like yours, will refuse to boot at all, one day.
