Unwanted Sound Byte

Whenever I have sound on, a voice keeps saying 'i'm sorry I did not understand'
this happens no matter what I am listening to. this happens even when error sounds occur in XP. I am sure there is a loop some where but I do not know where to look. Anybody have any ideas? any help would be appreciated.
