Rootkit.TDSS infection

I've been receiving help at the "Am I infected? What do I do?" forum. After running scans with RootRepeal and GMER, my helper advised me that I have a rootkit on my machine and instructed me to come over here for additional help. At this stage, the main issue is that Malwarebytes' Anti-Malware is continually identifying "Rootkit.TDSS" on my machine but when I remove it and reboot, it is back. I can't say what the symptoms are aside from the computer running extremely slow. Since I've been through so many problems previous to this, I have not been using the computer for any normal operation and, therefore, don't know if anything else is being affected.

Per the instructions in the "Preparation Guide for use before posting about your potential Malware problem" thread, I am pasting my DDS log and attaching the attach.txt and ark.txt files in one zip file. Thank you for your assistance.

This thread can be closed. My wife decided to bite the bullet and do a reinstall of Windows. Luckily we were able to save her important files, so no harm done. Thanks to Blade Zephon for helping me in my previous thread.
