NTOSKRNL- HOOK infection

I have a very bad infection of the NTOSKRNL - HOOK rootkit.d trojan. McAfee finds it, says it cleans it but of course it does not. I tried downloading Antimalwarebytes and Superantispyware, but they won't run. Scan shows even my F: partition (which I've never used) which holds my recovery disk is infected. I've backed up what I think I need onto USB memory stick, but assume that is suspect as well. Don't know anything about these so help is needed. I'm running Vista home premium SP1 on a Dell Inspiron (less than 2 yrs old so I really need to salvage it).
