Provided the source of the theme is a trusted one, then yes.
Under Windows XP (which I'm assuming you're using) it is not possible to install customized themes that haven't been digitally signed by Microsoft without patching the uxtheme.dll
file. Luckily, most themes available for download come with a utility to do this automatically.

Currently, I use Royale Noir
, a variant of the Royal Blue
theme. These themes were created by Microsoft but never officially released.
