After OS Reinstall, can't read SD Card

I recently got a new hard drive and I decided not to transfer all the old stuff from my original hard drive. Anyway, now my computer won't read my Sandisk SDHC card. It's weird, because before the changes, it was reading it fine. I'm not sure if I need to get a new driver, or what. The details of my current driver are...
